Payment
If you opt in by checking the box, your Key Vision Plan membership will automatically renew each year at the then-current annual membership rate plus the $10 non-refundable processing fee.
Your payment method on file will be charged the applicable annual membership fee plus the $10 processing fee on your renewal date each year, until you cancel. The annual rate is the then-posted membership rate for your selected plan at the time of renewal.
You may cancel auto-renewal at any time by logging in to your Member Dashboard or by calling us at (800) 539-9563. To avoid being charged for the next annual term, your cancellation must be received before your renewal date.
Membership fees are refundable within thirty (30) days after your effective date or receipt of membership materials (whichever is later), provided the plan has not been utilized at a participating provider, less a nominal processing fee. The nominal processing fee is $5 for Maryland residents; Arkansas residents will be refunded the processing fee. The $10 processing fee is otherwise non-refundable. If the plan has been utilized, the membership is non-refundable for a minimum one-year term.
Key Vision Plan will accept cancellation of auto-renewal at any time and will cease collecting membership fees no later than 30 days after receiving a valid cancellation notice. Written cancellation requests may be sent to: Member Services, Key Vision Plan, 424 Rosevale Avenue, Ronkonkoma, NY 11779, or faxed to 631-272-5231.
By signing up, you indicate that you have read the terms and conditions of the plan and you authorize Key Vision Plan to continue to charge your payment method for the price of your membership, unless and until you cancel in writing.
